By signing this form, I certify that my health is in good condition to permit me to participate in Yoga training in the Third Annual Ann Arbor Rotary Yoga Retreat on Sunday, June 14, 2026 at The Kensington Hotel, 3500 S State St, Ann Arbor, MI. I hereby acknowledge that a certain amount of risk is involved in my participation in the program activities of the Yoga Retreat. I agree to assume all risk and take personal responsibility for any injuries and illness associated with my participation in the activities of the Third  Annual Ann Arbor Rotary Yoga Retreat.

I further expressly agree to release and indemnify Rotary Club of Ann Arbor, its officers, employees and all Ann Arbor Rotary Yoga Retreat faculty members and volunteers and anyone else associated with the Ann Arbor Rotary Yoga Retreat from any and all claims, demands and damages of whatsoever arising from my attendance and participation in the Ann Arbor Rotary Yoga Retreat, whether known or unknown, anticipated or unanticipated, which I may have now or in the future, including costs and legal fees, but not limited to any and all claims.

I understand the images and video of the faculty and participants may be recorded during the Ann Arbor Rotary Yoga Retreat, and I give permission to record and distribute my images and video for educational purpose.
